The Concurrent Supportive Housing (CSH) Program provides support coordination and counselling for clients who have both mental health and substance use issues. Support may include life skills/functional support, support to secure and maintain housing, recreation opportunities, system navigation and referral, screening and assessment, support coordination, and direct counselling. This program is a partnership between Thresholds Homes and Supports (THS) and House of Friendship (HOF). Support is provided by an integrated team comprised of Addiction Counsellors and Mental Health and Addiction Coordinators. It is an expectation of the program that participants will connect with their supports each week and is designed for clients who have made the choice to eliminate or change their substance use in order to increase quality of life and reduce risk caused by substance use. The program strives to improve the health and social outcomes of people with concurrent mental health issues by providing individualized support, based on identified goals related to each person’s vision of a happy, hopeful life.
18 years and older
Must live in the Region of Waterloo or County of Wellington
Must have a diagnosed or diagnosable severe and persistent mental illness
Substance use concern
Use of a substance that affects at least 4 of 8 of the following domains:
Activities of daily living
Finances
Social relationships
Housing
Physical health
Employment
Justice involvement
Use of emergency services
Individual must be motivated to make changes to their substance use. Does not need to have abstinence as goal; could be harm reduction
Willing and motivated to engage with support staff on a weekly basis
Currently, we can only take on individuals who have stable housing. Individuals who are eligible but need housing will remain on the waitlist for when housing becomes available.
