Case Management is a service provided to individuals who require mental health supports but do not require direct housing assistance from Thresholds. The service offers individualized supports with a strength based focus. Emphasis is placed on working with people to coordinate services that they receive as well as focusing on individual recovery goals, maximising independence and improving wellness and quality of life. Activities with staff can include:
building formal and informal supports in the community
helping people to find, coordinate and access the supports they need
planning and service related to crisis situations
personal goal setting and recovery planning
